Job SummaryWe are seeking a compassionate and dedicated Support Worker to join our team. This role involves assisting clients in their daily activities, ensuring their comfort, safety, and well-being while promoting independence.

Responsibilities

• Provide patient care and support to individuals with disabilities in various settings, including home care and assisted living environments.
• Assist clients with daily living activities such as meal preparation, personal hygiene, and medication administration.
• Implement care plans tailored to each clients needs, including case management and monitoring of progress.
• Maintain accurate documentation of client care and progress, adhering to medical terminology standards.
• Collaborate with healthcare professionals to ensure comprehensive support for clients, including hospice care when necessary.
• Foster a positive and encouraging environment that promotes the dignity and independence of each individual.
• Respond promptly to client needs and emergencies while maintaining a calm demeanor.

Requirements

• Proven experience in patient care or working with persons with disabilities is preferred.
• Knowledge of case management practices and medical terminology is highly desirable.
• Experience in medication administration and familiarity with home care practices is a plus.
• Strong communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
• Ability to work independently as well as part of a team in a fast-paced environment.
• Compassionate nature with a genuine desire to help others improve their quality of life.
• Flexibility in working hours may be required based on client needs.

Pay: $19.00-$22.00 per hour

• 8 hour shift
• Day shift
• Evening shift
• Monday to Friday
• Morning shift
• On call
• Weekends as needed
