They play a critical role in ensuring the well-being of residents, monitoring their conditions, and providing compassionate and skilled care.**What You Will Do*** In collaboration with the Director of Care/Assistant Director of Care, implement AgeCare policies and procedures on the neighbourhood. Provide input into the revision of procedures within the Home.* Liaises with physicians and other members of the health care team including external health care partners. Immediately notifies the physician of any significant changes in resident status.* Coordinate nursing and other services for the residents within the neighbourhood and with other services within the residence.* Provide supervision and direction to the Registered Practical Nurses and Personal Support Workers.* Lead shift report with members of the health care team, ensuring that all team members are informed of changes in residents’ status.* Record significant changes in residents’ condition; ensures documentation in the residents’ chart adheres to AgeCare policies and procedures and Provincial College of Nurses and Standards of Practice.* Review and process physician orders as per procedure, ensuring orders are implemented as soon as possible.
