Position overview We are seeking an experienced Community Worker to deliver structured community and welfare support services to clients in home and community settings. This role focuses on assessing needs, coordinating supports, delivering participation programs, and connecting clients with appropriate services and resources to achieve their individual goals. Key responsibilities Assess individual client needs, circumstances, strengths, and support requirements through direct engagement and ongoing review Plan, develop, and implement individual support strategies that promote independence, inclusion, and wellbeing Support clients to access health, housing, education, training, employment, and community based services Assist clients to participate in community, social, recreational, and daily living activities aligned with their goals Monitor client progress and wellbeing, identifying changes in needs and responding appropriately Liaise with families, carers, community organisations, and service providers to coordinate and integrate supports Provide information, guidance, advocacy, and referrals to relevant welfare and community services Maintain accurate case notes, progress records, and service reports in line with organisational and funding requirements Contribute to service planning, outcome reporting, and continuous improvement activities Deliver services in a professional, respectful, and culturally sensitive manner at all times Skills and experience
