The Social Worker counsels and provides guidance to individuals and/or families with various social, medical and/or psychiatric challenges, under the guidance of the Director of Care.

Responsibilities

• Helps resident and family members through individual or group conferences to understand, accept, and follow medical recommendations
• Supports residents and families with problems concerning relationships or other aspects of social functioning affected by disability
• Collaborating with the multidisciplinary teams in diagnosis and treatment plans
• Conducts psychosocial screening activities determining required specialized psychiatric assistance
• Serves on committees as required
• Performs other related duties as assigned

Qualifications and Experience

• Must be a Registered Social Worker or eligible for registering
• Bachelor’s degree in social work, or one of the following Counseling, Psychology, Behaviour science or related field
• Minimum 2 years experience working in a social services field or related capacity; or any equivalent combination of experience and/or education from which comparable knowledge, skills and abilities have been achieved.
• Preferred experience in a Long Term care setting.
• Knowledge of the principles, practices, techniques, and professional standards in the field of social work
• Understanding of community organizations and social service programs available
• Strong counselling and conflict resolution skills
• Exceptional interpersonal skills with peers, residents, visitors, and operational partners
• Solid organizational skills, demonstrate the ability to prioritize and problem solve
